    Hello,

    I am finding your website useful and I am grateful for the work you have done to provide all of us with these resources.
    I attempted to fill out the form to obtain the EKG cards, however it requests a US state before it can be submitted. The issue is that I live in Ontario, Canada. If you are able to send these cards to Canada, how would you like me to fill out the form.
